Creating the Hospital of the Future: The Implications for Hospital-Focused Physician Practice |
The adoption of a hospital-focused model of practice by many medical specialties is changing the essence of care delivery in the inpatient setting. Join your physician and hospital CEO colleagues for this complimentary program to hear from some of the leading edge experts on the evolving nature of hospital-based medicine. The American Hospital Association’s (AHA) Physician Leadership Forum and the Society of Hospital Medicine (SHM) will host a special post-Health Forum/AHA Leadership Summit session on creating the hospital of the future on Saturday, July 21 from 11:30 a.m. – 5:30 p.m. at the Marriott Marquis in San Francisco, CA. This interactive half-day session will include a review of the changing face of hospital-based physician practice, a view from the CEO perspective on the impact of hospital-focused medicine on delivery of care and the bottom line, and how hospital-based practice can lead to better performance and safer hospital care. The session is structured to encourage networking and exchange of ideas among attendees, and will include presentations, panel discussions, and Q&A sessions. The Hospital of the Future meeting is brought to you through collaboration between the AHA and SHM and generously sponsored by Apogee Physicians, Delphi Healthcare Partners, and Eagle Hospital Physicians. Attendance is limited to ensure opportunity for dialogue. Webinar on managing the unique needs of different patient populations set for May 16 |
Register now for the next PLF webinar, "Managing the Unique Needs of Different Patient Populations," taking place Wednesday, May 16 beginning at 3 p.m. ET. This free, 90-minute webinar focuses on two unique approaches to the role of care coordination among different patient populations. The session will begin with an overview of Access DuPage, a program in DuPage County in Illinois that’s a collaborative effort by hundreds of individuals and organizations to provide access to medical services to the county’s low-income and medically uninsured residents. Richard Endress, President, DuPage Health Coalition, will discuss how this program is able to coordinate care through the use of medical homes for a patient population set with unique needs while at the same time containing costs. Rahul Koranne, MD, MBA, FACP, Medical Director, HealthEast Bethesda Hospital, will next discuss how Bethesda Hospital, a 145-bed not-for-profit freestanding long-term acute care hospital in St. Paul, Minn., cares for a large population of Medicare and Medicaid recipients and how they are able to effectively manage their care and reintegrate them back into the community by coordinating with local health care and social agencies. |

IOM recommends HHS promote integration of primary care, public health |
The traditional separation between primary health care providers and public health professionals is impeding greater success in meeting their shared goal of promoting population health, according to a report released today by the Institute of Medicine (IOM). The report recommends the federal Centers for Disease Control and Prevention and Health Resources and Services Administration foster integration between primary care and public health through funding, policies and other means. Based on a review of published papers and case studies in specific cities, the study committee found that successful integration of primary care and public health requires community engagement to define and tackle local population health needs; leadership that bridges disciplines and jurisdictions and provides support and accountability; shared data and analyses; and sustained focus by partners. |

CMS hosting webinar on interventions to reduce avoidable hospitalizations |
The Centers for Medicare & Medicaid Services (CMS) will host a webinar on reducing avoidable hospitalizations among nursing home residents on Tuesday, April 17 at 2 p.m. ET. During this 90-minute webinar, independent faculty will give their opinions on successful strategies to reduce such hospitalizations. |

Choosing Wisely campaign hopes to curb overuse of certain tests/procedures |
Nine medical societies, including the American Academy of Family Physicians and the American College of Physicians, have identified 45 tests or procedures that may be overused or unnecessary for certain patients based on the evidence and encourage doctors to perform these tests/procedures less often. The Choosing Wisely campaign is an initiative of the American Board of Internal Medicine Foundation, an affiliate of the American Board of Internal Medicine, and in partnership with Consumer Reports. |

CMS adds 27 providers to Medicare Shared Savings Program |
The Centers for Medicare & Medicaid Services (CMS) recently announced the addition of 27 health care providers to the Medicare Shared Savings Program, effective April 1. These new ACOs join the first 32 Pioneer ACOs, which CMS announced last December. |
