Project BOOST

The Society of Hospital Medicine is leading a national initiative called Project BOOST (Better Outcomes for Older adults through Safe Transitions), a program designed to improve the care of patients as they transition from the hospital back to their homes. By improving hospital discharge processes, Project BOOST aims to:

  • Reduce 30-day readmission rates for general medicine patients (with particular focus on older adults)
  • Improve patient satisfaction scores and H-CAHPS scores related to discharge
  • Improve flow of information between hospital and outpatient physicians and providers
  • Identify high-risk patients and target specific interventions to mitigate their risks for adverse events
  • Improve patient and family preparation for discharge

Download a free copy of our guide on team-based care
The AHA’s Physician Leadership Forum is pleased to share "Team-Based Health Care: Lessons from the Field," a guide describing the core concepts of team-based care. The guide features case studies on how three different leading-edge health care organizations (AtlantiCare Health System, Brigham and Women’s Hospital, Marquette General Health System) approached fostering team-based care. While each organization approached team-based care differently, several common themes emerged – having strong leadership in place to lead the change, using data to drive/sustain the change, training/educating members to work as a team, and finding peoples’ passion to motivate them.

Register now for our webinar on physician leadership in clinical integration
Register now for the next PLF webinar, "Physician Leadership in Clinical Integration," taking place Wednesday, March 21 beginning at 3 p.m. ET. This free, 90-minute webinar features Jacque Sokolov, MD, chairman and managing partner of SSB Solutions, discussing key physician leadership opportunities in the rapidly expanding world of clinically integrated organizations (CIOs). Dr. Sokolov will review meaningful clinical integration and how it can drive physician alignment and lead to CIOs.

In his experience, Dr. Sokolov has found the one constant in developing a successful integrated organization is physician leadership at the governance, management and operating levels. In addition, the ability to deliver on value-based reimbursement contracts will require an ongoing clinical model that is significantly different than today’s and will underscore the need for meaningful physician involvement.
